DOCTOR’S APPOINTMENT INFORMATION
  • Identity Documents – each student must carry a valid ID card or passport.
  • Occupational Health Questionnaire– it must be completed very carefully.
    https://cmr.gumed.edu.pl/medycyna-pracy/
  • Occupational Health Doctor’s Appointment – each student must bring:
    - proof of vaccination against Hepatitis B (HBV), or, if they do not remember whether they were vaccinated or are unsure
    - the result of an anti-HBs test (this test may be done in the student’s home country; results in English are accepted).
The result will allow the doctor to determine whether a booster is needed. In the case of no prior vaccination, it will be mandatory to complete the full vaccination course consisting of three doses. Vaccination can also be done on site at the GUMed Family Medicine Center.
  • Please inform us if there are any female students in the groups who, for religious reasons (e.g., students from Islamic countries), do not wish to be examined by a male doctor. These students will be scheduled with a female occupational health doctor.
Group A & B If you’ve just joined Orientation Week or arrive a little later, remember to get tested:
  • blood test in treatment room number 13, Mon-Fri, 7:30-9:45 AM.
  • doctor appointments, after having your blood drawn and completing the questionnaire, please email studentcmr@gumed.edu.pl – you will be scheduled for an appointment.

According to regulations, all medical students are required to undergo mandatory vaccination against hepatitis B (HBV). This vaccination protects against infection, which can occur through contact with blood or other biological material during medical training and future professional practice.

If you are not sure whether you have been vaccinated and do not have any medical records confirming it, please consult your family doctor (GP) before coming to the nurse’s appoitment, who whill advise whether vaccination is necessary.

If you are certain that you need to receive the vaccination, you are welcome to come to Centrum Medycyny Rodzinnej (CMR) to nurse Dorota Słodkowska on 20–24 October, between 12:00–13:00. At the appointment with the nurse, students must know whether they have been vaccinated before or not.

The qualification visit and vaccination are free of charge for students.
